Only the spell's target can see the killer, though you can see the vague shape of the illusion as it races forth to attack. I'm not sure I understand the question, but I'll try: Phantasmal Killer always hit, then the target can succed a will save to cancel it entirely (they recognise it's an illusion), then if they fail that they can succed a fortitude save to take 3 to 18 damage instead of dying. The effect of the killer is based on the outcome of the target's Will … You might scrub a firey salamander and re-define them as a brutal, humanoid porcupine warrior leading a band of mutated beast-folk.
